We are based in two locations across the North East of England:

  • North of Tyne
  • South of Tyne

We provide support to any child or young person from across the North East with a wide range of support programmes and activities. 


Assessments

In order to provide person centred services it is vital that practitioners have an in-depth, current, holistic assessment to inform their practice. The assessment will feed in to the service user’s personal profile, which will support staff to meet the needs of the service user. Evaluation and review will form an intrinsic part of the assessment which will feed into learning pathways creating more positive outcomes for the service user. This information will be shared with service commissioners to evidence service outcomes.
