The evidence that you reference must have taken place within the three year- period from August 2019 to August 2022. If the evidence is not within the time frame the nomination cannot be accepted. The responsibility is on the person nominating to assess the level of contribution and provide evidence of how it exceeds normal expectations and how that ‘extra contribution’ has had a benefit to services, staff, service users, learners, parents, carers or other family members. Without evidence of ‘going over and above’ and evidence of how that effort benefited one or more of the above, the nomination cannot be accepted. Please limit your nomination to 1000 words/minimum 250 required.
